Release of Records Northeast

Any student wanting to enroll at Northeast Elementary, who has attended in another school district, needs to download, complete, and return this form to the Main Office at Northeast.

Any student wanting to enroll at West Elementary, who has attended in another school district, needs to download, complete, and return this form to the Main Office at West.

Middle School Healthy Kids Act Form

In 2008, the Iowa Legislature enacted “the Healthy Kids Act,” requiring that all students in grades 6-12 engage in physical activity for a minimum of 120 minutes per week in which there are at least five days of school.  The law also requires that we monitor how students fulfill this requirement.

High School Healthy Kids Act Form

In 2008, the Iowa Legislature enacted “the Healthy Kids Act,” requiring that all students in grades 6-12 engage in physical activity for a minimum of 120 minutes per week in which there are at least five days of school.  The law also requires that we monitor how students fulfill this requirement.  If students do not get 120 minutes of weekly activity, we will need to schedule them in a PE class or a walking club that meets during seminar.
